Because he looked back before he had gotten out, Eurydice disappeared. It wasn’t necessarily a punishment but more of a consequence because he was foolish and hadn’t wait till he had got out.

Hades punished him for his lack of faith by keeping Eurydice dead.

Dionysus sent his Maenads to kill Orpheus to punish him for preferring to worship Apollo.
